Co-payments which appear in bold exceed 10% of household income and would be reduced by the amount which appears in parenthesis underneath the co-payment amount if a 10% cap were in place.

1. The state income standard (SIS) corresponds to the year 2001 federal poverty level and was effective 6/1/01. Co-payments are calculated by subtracting the state income standard from the household income, and multiplying the resulting number by a percentage chosen by the county of between 10% to 35%.
2. Counties with a 10% co-payment include: Cattaraugus, Chatauqua, Columbia, Livingston, Otsego, Schenectady, Schoharie.
3. Counties with a 17.5% co-payment include: Delaware, Nassau.
4. Counties with a 20% co-payment include: Allegany, Essex, Fulton, Madison, New York City, Ontario, Putnam, Saratoga, Steuben, Tompkins, Washington.
5. Counties with a 23% co-payment include: Albany.
6. Counties with a 25% co-payment include: Broome, Hamilton, Herkimer, Jefferson, Lewis, Monroe, Onieda, Orleans, Oswego, Rensselaer, Suffolk, St. Lawrence, Tioga, Ulster, Warren, Wayne.
7. Counties with a 30% co-payment include: Chemung, Franklin, Rockland, Wyoming.
8. Counties with a 33% co-payment include: Westchester.
9. Counties with a 35% co-payment include: Cayuga, Chenango, Clinton, Cortland, Dutchess, Erie, Genesee, Greene, Montgomery, Niagara, Onondaga, Orange, Schuyler, Seneca, Sullivan, Yates.
